
Valley Memorial Homes will be opening a Transitional Care Unit (TCU) at Valley Eldercare Center in the summer of 2010. Transitional care is for patients who have completed their acute stay in the hospital. The (TCU) will help your recovery progress as safely and quickly as possible. Our hope will be to return you to your previous living arrangement. Most TCU patients will require only a short stay before they are able to return home or to a lesser level of care.
Patients will be able to continue to receive IV therapy and wound dressing changes as well as physical, occupational, speech or respiratory therapies.
Our transitional care services include:
- Licensed Nurses
- Dedicated and compassionate caregivers
- Social Services
- Physical Therapy
- Occupational Therapy
- Speech Therapy
- Diet and Nutritional Counseling
- Exercise and Social Programming
- Spiritual Care
The TCU will consist of 32 private and 2 semi-private rooms. The unit will provide larger rooms with state of the art equipment.
The healthcare team will be available to help you restore your strength and re-establish your independence.
